As a kid, I always slept with my head above the blanket. Around 10 years of age I noticed my father sometimes sleep with the blanket covering his head, and ever since, that's how I sleep 99% of the time.

Sometimes I go hybrid mode where I situate the blanket to be simultaneously covering my head, but also get good airflow.
 
If its colder in the room I do undercover, can't sleep with a cold nose. Use a comforter and a blanket, put the comforter over your head and the blanket normal ear height and you get good air flow and good darkness.

Also if I'm somewhere like a crappy hotel with bad light coverage, also undercover.

My room can get kinda cold in the winter and I wish I could sleep with my head under the comforter, but I just can't. Like I feel like I'm not getting enough oxygen or something. I've even tried like lifting part of it to vent fresh air in, but the air coming in is cold so it kinda defeats the point.

As a kid definitely under, because you never know if the Bed Troll not might get adventurous and get out from under the bed on an evening, but it's been scientifically proven that head under Duvet and it's all good because Bed Trolls suffer from Duvet Blindness (it is known). However, given in this day and age the underside of the bed is full of bedding draws and the like, I'm slightly more comfortable with head above, though obviously duvet over the neck in case the Cupboard Vampires gets adventurous 🤔
 
Mostly above, but if it gets particularly cold and dry I'll either put the blanket up with an air gap, or just sleep with one of the many unnecessary cushions my wife has on my face.

Although when I do that, quite often the rabbit ends up sleeping on the cushion, which makes it tricky to sleep.
 
Depends on the season , it gets cold in the North East so in the winter I usually cover but leave enough of an area to get fresh air, the rest of the year not so much but I need to shut the blinds for complete darkness.
As a bit of boner...I mean bonus info in the summer I usually sleep naked with no covers or anything so I would suggest knocking before just walking into my room unless you want a big surprise.

Window open ice cold room and crisp fresh air, head above the cover but thick as fuck winter duvet with an extra sheepskin on top when needed. Sadly those glorious days are now long gone, my wife prefers to sleep in a sauna leaving me uncovered and in the nude just to avoid drenching our mattress in sweat.

When I'm actually sleeping my head is always above the duvet. But when it's really cold and I'm going to bed, I usually use a hairdryer to rapidly warm the area under the duvet. I then hop in and cover myself completely until I'm warmed up.

I like to turn one of these on, let it get warmed up, and then stick it in my ass to warm my body up from the inside while I sleep. I don't even need a blanket.

I won't speak for all of us here (1 of 2) but for me, has nothing to do with temperature.

On a serious note though, lack of oxygen during sleep may lead to morning headaches. I always sleep with head above blanket. I usually open windows hour before going to sleep to lower temperature, it helps me to fall asleep better.

This. Fresh air, then around 18C temperature at most, great sleep. I have a water bed, where the heating of the water makes it comfy as fuck, even when the room itself is cold.

Water bed? Does it support your back as good as traditional mattress?

No and that's kinda the point. You have to chose and adjust it to your weight, there are different kinds of water beds. My wife has a softer one while my side is way more "stable". A well chosen waterbed is better for the back, at least for me and my wife, it was a significant improvement.

For example, my wife sleeps on her side, this shows why the waterbed is better for her back:
