How do you trigger?

They need to shift the middle finger trigger button to under the controller where most people actually rest that finger. Which is I suppose why the paddles are on the pro controllers.

Depends on the genre. If I'm playing a first person shooter (console peasant here) I'm usually keeping my index finger on the trigger and occasionally moving up to the shoulder buttons to activate the weapon wheel or switch between items (or whatever).

But a lot of 3rd person action games use the triggers and shoulder buttons equally, so I put a finger on all four.
 
I think I fucked up my index fingers back in the PS2 days. So now sometimes I actually do use my middle finger on R2.

I don't like it, but if it's a game with driving or something, my index finger can't take it.

I use the index finger only. It feels weird as hell trying to hold a controller with just the back two fingers, to be honest, but it sits comfortably with three. I dare say if I played enough games that make use of both RB & RT at the same time I'd get used to it, but it's not something I've come across much.
